To prepare for your driver license test, you should study the Hawaii Driver's Manual prior to taking the driving test and you should become familiar with local traffic laws and the rules of the road. Manuals are available at local bookstores and State libraries. Form BMV 5745 (Application Information for Standard/Compliant Ohio Driver License or ID Card) may be used to prove SSN only if the applicant has previously established their SSN with the BMV. This form is also available at Deputy Registrar agencies to be filled out at the time of the transaction. Applying for a Duplicate (Replacement) License If your driver license has been lost, stolen or destroyed, you may be eligible to obtain a replacement online. - If you lose or misplace your Montana driver license, you may obtain a replacement license from any driver license exam station. To obtain a replacement license, you must provide proof of identity. If not, your duplicate will be marked 'NOT FOR OFFICIAL FEDERAL USE.'
A Real ID will be required for those who wish to use a driver's license or state ID to board aircraft beginning May 3, 2023. See DHS Real ID Information for details and other instances where a Real ID is required.

The Department is authorized to waive the fee for homeless persons 25 and older to obtain a duplicate Nevada driver's license or ID card. This is a one-time exemption for duplicates only. You must apply for this in person at a DMV office and complete the declaration form.
You may be required to reimburse the $3.25 photo fee to the Department at the time of your next regular renewal if you are employed at the time of the renewal.
The fee exemption does not apply to:
- New residents obtaining a Nevada license or ID for the first time
- Renewals
- Any fees or requirements associated with a license suspension or revocation
- Commercial licenses
Under 25 Years of Age
The Department is authorized to waive the fee for homeless persons under 25 to obtain an original, renewal or duplicate Nevada driver’s license or ID card. This is a one-time exemption. You must apply for this in person at a DMV office and complete the declaration form.
